Effects of Second Hand Smoking

As if the growing number of smoking bans in restaurants, airplanes and other public places isn't sending a strong enough message, researchers now have the first biological data confirming the health hazards of secondhand smoke.

Scientists led by Dr. Ronald Crystal at Weill Cornell Medical College documented changes in genetic activity among nonsmokers triggered by exposure to secondhand cigarette smoke. Public-health bans on smoking have been fueled by strong population-based data that links exposure to secondhand cigarette smoke and a higher incidence of lung diseases such as emphysema and even lung cancer, but do not establish a biological cause for the correlation. Now, for the first time, researchers can point to one possible cause: the passive recipient's genes are actually being affected. (See a new recipe for longevity that says no to smoking.)

Crystal's team devised a study in which 121 volunteers — some of whom smoked and some of whom had never smoked — agreed to have samples of their airway cells studied for genetic activity. The subjects also provided urine so the researchers could measure the amount of nicotine and its metabolites, like cotinine, for an objective record of their exposure to cigarette smoke.

Airway cells that line the bronchus, from the trachea all the way to the tiny alveoli deep in the lungs, are the first cells that confront cigarette smoke, whether it is inhaled directly from a cigarette or secondhand from the environment. Crystal's group hypothesized that any deterioration in lung function associated with cancer or chronic obstructive pulmonary disease, including emphysema and bronchitis, in which the lungs lose their ability to take in air, would begin with these cells. And indeed, that's what he and his team found. The researchers removed airway cells from the volunteers using a bronchoscope and tested all 25,000 identified human genes in them to determine which ones were active — either turned on or off — in response to cigarettes. They narrowed the search to 372 genes that were active among the smokers but not in the cells of the nonsmokers. Based on the level of nicotine in the urine, the scientists also divided the volunteers into three groups: smokers, who showed the highest level of the tobacco metabolites; nonsmokers, who showed none of these compounds and a low-exposure group who fell in between. Comparing the 372 genes among these three groups, they found that the low-exposure group shared 34% of the same active genes with nonsmokers and 11% of the same gene activity with smokers. The low-exposure group included both nonsmokers who have never lit up as well as those who admitted to smoking only occasionally.

The results suggest that the genetic changes among the low-exposure volunteers, some of whose exposure is exclusively secondhand, mimicked those of smokers and represent the first molecular steps toward later lung disease. The study did not follow the subjects long enough to document what effect the genetic changes may actually have on the lung tissue, but Crystal says those studies are forthcoming.

"What is interesting to me is how sensitive the lung cells are to any cigarette smoke," he says. "It doesn't matter if you are walking into a cocktail party where other people are smoking or if you smoke one cigarette a week. No matter what level of exposure you have, your lung cells know it and they are responding. It's sort of like canaries in the coal mine — they are crying out and saying, 'I'm changing here, I'm changing the genes that I turn on and off in response to this environmental stress.'" It's not clear how permanent these genetic changes are, but previous data suggests that, at least in smokers, some of the alterations may be irreversible. Smokers experience a decline in lung function that is accelerated compared with nonsmokers, and even if they kick the habit, they can never achieve the same level of function as those who never lit up. If the genetic results are confirmed, says Crystal, they may help doctors to identify those whose genetic makeup put them at higher risk of developing lung disease when exposed to cigarettes, and potentially steer them toward drugs that can help them suppress the dangerous effects of nicotine on their cells.

In the meantime, the latest findings should reinforce public-health messages about the dangers of cigarette smoke, even if it is secondhand, says Dr. Norman Edelman, chief medical officer of the American Lung Association. "When you look at the biology, there is no safe level of exposure to tobacco smoke," he says. "This [study] adds an important piece of evidence that inhaling secondhand smoke is deleterious and does things to the airway that are not good."

SENSORY FUNCTION, TESTING STRETCH OR DEEP TENDON REFLEXES

Sensory Function

>>Sensation is tested by evaluating the patient’s ability to perceive a light touch, superficial pain (pin-prick), differences in temperature, vibration, position sense and motion. If any abnormality is found, it is important to identify the area of deficit clearly and find the point where the abnormal sensation becomes normal again. This point is referred to as a sensory level.

>>Touch the patient in various areas with cotton (light touch) and with the tip of a pin (pin-prick)

>>Typically begin with the face and move down the body, noting any asymmetry between the right and left sides.

Sensory Function: Stereognosis
-Test the person’s ability to recognize objects by feeling their forms, sizes and weights.
-With eyes closed, placed a familiar object (paper clip, key, coin, cotton ball, or pencil) in the person’s hand and ask the person to identify it.
-Normally, a person will explore it with the fingers and correctly name it.
-Testing the left hand assesses right parietal lobe functioning.
-Astereognosis – inability to identify object correctly. Occurs in sensory cortex lesions e.g. stroke

Sensory Function: Graphestesia

-The ability to “read” a number by having it traced on the skin.
-With the person’s eyes closed, use a blunt instrument to trace a single digit number or a letter on the palm.
-Ask the person to tell you what it is.
-

Inability to distinguish number occurs with lesions of the sensory cortex

Testing Stretch or deep Tendon reflexes

-Evaluation of deep tendon reflexes (DTRs) reveals the intactness of the spinal reflex arc at specific spinal levels as well as the normal override on the reflex of the higher cortical levels.
-DTRs are usually tested by tapping on a tendon with fingers or a reflex hammer. This causes a stretching of certain muscles and results in contraction. When damage occurs to higher centers (upper motor neurons), the spinal reflex arc is uninhibited and the DTRs are hyperactive.

Grading Scale – Reflex (4 Point Scale)-Reflexes are graded on a scale of 0 to 4.

-4+ Very brisk, hyperactive with clonus, indicative of disease
-3+ Brisker than average, may indicate disease
-2+ Average, normal-1+ Diminished, low normal
-0 No response

Definition of terms:

Clonus – is a set of short jerking contractions of the same muscle, is a repeated reflex muscular movements. A hyperactive reflex with sustained clonus (lasting as long as the stretch is held) occurs with upper motor neuron disease.
Hyperreflexia – is the exaggerated reflex seen when the monosynaptic reflex arc is released from the influence of higher cortical levels. This occurs with upper motor neuron lesions. e.g. stroke
Hyporeflexia – which is the absence of a reflex, is a lower motor neuron problem. It occurs with interruption of sensory afferents or destruction of motor efferents and anterior horn cells e.g. spinal cord injury

Dermatomes
-A circumscribed skin area that is supplied mainly from one spinal cord segment through a particular spinal nerve.
-The dermatomes overlap, which is a form of biologic insurance. That is, if one nerve is severed, most of the sensations can be transmitted by one above and the one below.

Dermatomes Landmarks
-The thumb, middle finger and fifth finger are each in the dermatomes of C6, C7 and C8.
-The nipple is at the level of T4.
-The umbilicus is at the level of T10.
-The groin is in the region of L1.

Biceps Reflex (C5 to C6)

-Support the person’s

forearm on yours; this position relaxes, as well as partially flexes, the person’s arm.
-Place your thumb

on the biceps tendon and strike a blow on your thumb.
-You can feel as well as see the normal response, which is contraction of the biceps muscle and flexion of the forearm.

Triceps reflex (C7 to C8)

-Tell the person to let the arm “just go dead” as you suspend it by holding the upper arm. Strike the triceps tendon directly just above the elbow.
-The normal response is extension of the forearm.
-Alternately, hold the person’s wrist across the chest to flex the arm at the elbow, and tap the tendon.

Brachioradialis reflex (C5 to C6)

-Hold the person’s thumbs to suspend the forearms in relaxation.
-Strike the forearm directly, about 2 to 3 cm above the radial styloid process.
-The normal response is flexion and supination of the forearm.

Quadriceps reflex “Knee jerk” (L2 to L4)

-Let the lower legs dangle freely the knee and stretch the tendons.
-Strike the tendon directly just below the patella.
-Extension of the lower leg is the expected response.
-For the person in the supine position, use your own arm as a lever to support the weight of one leg against the other leg.
-This maneuver also flexes the knee.

Achilles reflex “Ankle jerk” (L5 to S2)

-Position the person with the kn

ee flexed and the hip externally rotated.
-Hold the foot in dorsiflexio

n, and strike the Achilles tendon directly.
-Feel the normal response as the foot plantar flexes against your hand.
-For the person in the supine position, flex one knee and support that lower leg against the other leg so that it falls “open.”
-Dorsiflex the foot and tap the tendon.

Disorders that involve physical symptoms

Conversion Disorders - anxiety converted into a physical symptom (a là Freud)
Hypochondriasis - normal sensations interpretedas symptoms of scary sicknesses

Somatization is the transference of mental experiences and states into bodily symptoms.
Somatoform disorders are the presence of physical symptoms that suggest a medical condition without a demonstrable organic basis to account fully for them.

☺The three central features of somatoform disorders:

>Physical complaints suggest major medical illness but have no demonstrable organic basis
>Psychological factors and conflicts seem important in initiating, exacerbating, and maintaining the symptoms
> Symptoms or magnified health concerns are not under the client’s conscious control

☺Five specific somatoform disorders:

>Somatization disorder: multiple physical symptoms; combination of pain, GI, sexual, and pseudoneurologic symptoms
>Conversion disorder: unexplained deficits in sensory or motor function associated with psychological factors; attitude of la belle indifference (lack of concern or distress)
>Pain disorder: has primary physical symptoms, pain unrelieved by analgesics; psychological factors influence onset, severity, exacerbation, and maintenance
>Hypochondriasis: preoccupation with the fear that one has a serious disease (disease conviction) or will get a serious disease (disease phobia)
>Body dysmorphic disorder: preoccupation with imagined or exaggerated defect in physical appearance

Ω Onset and Clinical Course
* Symptoms usually onset in adolescence or early adulthood
* All the somatoform disorders are either chronic or recurrent
* Clients will go from one physician or clinic to another, or they may see multiple providers at once in an effort to obtain relief of symptoms

Related Disorders
>> Malingering is the intentional production of false or grossly exaggerated physical or psychological symptoms; it is motivated by external incentives such as avoiding work, evading criminal prosecution, obtaining financial compensation, or obtaining drugs

>> Factitious disorder occurs when a person intentionally produces or feigns physical or psychological symptoms solely to gain attention
(In malingering and factitious disorders, people willfully control the symptoms. In somatoform disorders, clients do not voluntarily control their physical symptoms.)
>> Munchausen by proxy occurs when a person inflicts illness or injury on someone else to gain the attention of emergency medical personnel or to be a “hero” for saving the victim

ΩCultural Considerations
>> Somatization disorder is rare in men in the U.S. but more common in Greece and Puerto Rico
>> Men in India often have that, which is a hypochondriacal concern about loss of semen
>> Koro occurs in Southeast Asia and may be related to body dysmorphic disorder
>> Falling-out episodes, found in the southern U.S. and the Caribbean islands, are characterized by a sudden collapse
>> Hwa-byung is a Korean folk syndrome attributed to the suppression of anger and includes insomnia, fatigue, panic, indigestion, and generalized aches and pains
>> Shenjing shuariuo occurs in China and includes multiple symptoms

☺Etiology
1. Psychosocial theories:
* Unconsciously expressing internalized stress through physical symptoms (somatization)
* Primary gains are achieved when the direct external benefits of being sick provide relief of anxiety, conflict, or distress
* Secondary gains are obtained when the person receives internal or personal benefits from others because one is sick

2. Biologic theories:
* Familial tendencies
* Differences in the way body stimuli are regulated and interpreted

∞Treatment∞
 Treatment is focused on managing symptoms, improving quality of life, and improving coping skills
 Antidepressants are sometimes used for accompanying depression
 Referral to a pain clinic is helpful in pain disorder
 Involvement in therapy groups to improve coping and express emotions verbally has shown some benefit

Application of the Nursing Process

Assessment
 Investigate the client’s physical health status to thoroughly rule out underlying pathology requiring treatment
 History: client likely provides a detailed medical history; quite distressed about his or her health status (except the client with conversion disorder, who displays la belle indifference)
 General appearance and motor behavior: normal
 Mood and affect: may be labile, shifting from sad and depressed (describing physical ailments) to bright and excited (describing trips to health care providers)
 Thought processes and content: intact; content is about physical symptoms; vague in their description but use colorful, exaggerated terms
 Sensorium and intellectual processes: alert and oriented
 Judgment and insight: little or no insight; judgment may be affected by exaggerated responses to physical health concerns
 Self-concept: low self-esteem, lack of confidence, difficulty coping
 Roles and relationships: difficulty fulfilling family roles; few friends or social activities; may report lack of family support
 Physiologic and self-care concerns: legitimate health concerns may include disturbed sleep patterns, poor nutrition, lack of exercise, overuse of prescription medications

Data Analysis
Nursing diagnoses include:

-An emphasis on deference, passivity, and politeness should not be confused with a dependent personality disorder
-Cultures that value work and productivity may produce citizens with a strong emphasis in these areas; this should not be confused with obsessive-compulsive personality disorder
-Social stereotypes about gender roles and behaviors can influence diagnosis of certain personality disorders

Treatment

-Many people with personality disorders do not seek treatment because they don’t believe they have a problem
-Individual and group therapy may be helpful to those desiring change, but any changes are slow
-Improvement in relationships, improved basic living skills, relief of anxiety may be goals of therapy
-Cognitive-behavioral techniques such as thought-stopping, positive self-talk, and decatastrophizing can be effective

Pharmacologic treatment is based on the type and severity of symptoms rather than the particular personality disorder itself.

Four symptom categories include:

Cognitive-perceptual distortions including psychotic symptoms
Affective symptoms and mood dysregulation
Aggression and behavioral dysfunction
Anxiety

Pharmacologic Treatment for Symptoms

Cognitive-perceptual disturbances (magical thinking, odd beliefs, illusions, suspiciousness, ideas of reference, and low-grade psychotic symptoms)
-Low-dose antipsychotic medications
Mood dysregulation (emotional instability, emotional detachment, depression, and dysphoria)
-Lithium, carbamazepine (Tegretol), valproate (Depakote), low-dose neuroleptics, SSRIs, MAOIs, atypical antipsychotics
Aggression (predatory or cruel behavior, impulsivity, poor social judgment, and emotional lability)
-Lithium, anticonvulsant mood stabilizers, benzodiazepines, and low-dose neuroleptics
Anxiety
-SSRIs, MAOIs, or low-dose antipsychotics

Individual and Group Psychotherapy

Focus is on building trust, teaching basic living skills, providing support, decreasing distressing symptoms, and improving interpersonal relationships.

-Cognitive-behavioral therapy

-Basic living skills for people with cluster A personality disorders

-Inpatient hospitalization to provide safety for people with borderline personality disorder

-Assertiveness training groups for people with cluster C personality disorders

While I am on net surfing for some websites and updating some of my accounts like my Nuffnang Account, I was able to see this blogging contest "Enervon Hapontukin Contest" and I was curious about it that's why I opened it, then while reading about the content I was able to remember my old group mates in hospital duties and how we fight hapontukin. That's why I decided, to join the contest.

Ω Sexuality Ω

Sexuality is determined by anatomy, physiology, psychology, the culture in which one lives, one’s relationship with others and developmental experiences through out the life cycle. It includes the perception of being male or female and all those thoughts, feelings, and behaviors connected with sexual gratification and reproduction, including the attraction of one person to another. –sadock & sadock,2003

ΩSex – is described as one of four primary drives that also include thirst, hunger, and avoidance of pain.

Ω Sexual act – occur when behaviors involve the genitalia and erogenous zones.

Ω Sexuality is the result of biologic, psychological, social, and experiential factors that mold an individual’s sexual development, self-concept, body image, and behavior.

+Sexuality depends on four interrelated psycho sexual factors.

1. Sexual identity: whether one is male or female based on biologic sexual characteristics.

2. Gender identity: how one views one’s gender as masculine or feminine; socially derived from experiences with the family, friends and society.

HETEROSEXUALITY

TRANSVESTITE

3. Sexual orientation: how one views one’s life in terms of being emotionally, romantically, sexually or affectionately attracted to an individual of a particular gender. (e.g heterosexual [opposite sex], homosexual [same sex], bisexual [both sexes]).

Transgender is term used to define transsexuals and transvestite.

> Transsexual – individual whose sexual identities are entirely with the opposite sex.

Human Sexual Response Cycle

Sexual response – is a psychophysiologic experience.

Four phases:

Desire – is described as the ability, interest, or willingness to receive sexual stimulation

1. Excitement – sexual or arousal occurs as the result of physical or psychological stimulation. (sight,sound, thought)

2. Plateau-reached before orgasm. The vagina becomes extremely congested. The vasocongestion leads to the full distention of the penis.

3. Orgasm – “climax” phase.
* Women experience several strong, rhythmic contractions of the vagina. The vagina enlarges, the uterus contracts irregularly, increase v/s and the sex flush peak.
* Men experience emission and ejaculation of seminal fluid. Ejaculatory contraction involve the entire length of the penis.

4. Resolution – the organs and body systems gradually return the unaroused state.
- Vital signs return to normal.
- Relaxation and satisfaction are felt

+Predisposing factors for alterations

1. Biological / hormonal – may be associated with medical disorders (DM, neurological disorder)

2. Psychological

a. sexual trauma: rape. Abuse

b. intense emotions

3. Relationships

a. loss of interest in partner
b. marital conflicts
c. fear of commitment

+Clinical symptoms and diagnostic characteristics of sexual disorder Categories:

A. Sexual dysfunction disorder – involves an impairment of the sexual physiologic response or a disorders involve a disturbance in the processes that characterize the sexual response cycle, or the presence of pain during sexual intercourse.

Types:
1. Sexual desire disorders
Hypoactive – the lack of desire causes distress to the client or the client ‘s partner.
- Factors such as age, health, or diminished libido and lifestyle.

Sexual aversion disorder is used if anxiety, fear, or disgust occurs when an individual is confronted with sexual opportunity.

2. Sexual arousal disorder

3.Orgasmic disorders – are used to describe recurrent , persistent inhibited orgasm after an adequate phase of sexual excitement in the absence of any organic cause.

- Psychological factors are associated with female orgasmic disorder.
- Male orgasmic disorders may include premature ejaculation.

4. Sexual pain disorder

- This typically include dyspareunia and vaginismus.
- The diagnosis of dyspareunia is used to describe recurrent, persistent genital pain in the female and male that occurs during or after intercourse and is not used due to a general medical condition.
- Dyspareunia in men is uncommon and is usually associated with Peyronie’s disease- the presence of sclerotic plaques on the penis that cause penile curvature.

5. Gender identity disorder – discomfort with one biological sex (transsexualism)

Ω Paraphilias Ω

An emotional disorder characterized by unusual sexual behavior, including the practice of fetishism and sexual activities with unconsenting humans, or involving suffering and humiliation

A person may experience more than one paraphiliac disorder at the same time or may exhibit clinical symptoms of other mental disorders (personality disorder, schizophrenia)

+Characteristics of paraphiliacs

1. Emotional immaturity
> Seen in pedophile or “peeping Tom” who is unable to engage in a mature sexual relationship because of feelings of inadequacy

2. Fear of a sexual relationship that could result in rejection

3. Shyness ( seen in the voyeur who views others from a distance)

4. The need to prove masculinity, demonstrated by exhibitionist

5. The need to inflict pain on another to achieve sexual satisfaction

6. The need to endure pain to achieve sexual satisfaction

7. Low or poor self concept

8. Depression

NOTE: Not all of the characteristics listed are present in each paraphiliac.

Paraphilias

1. Bestiality (Zoophilia): sexual contact with animals serves as a preferred method to produce sexual excitement.

2. Exhibitionism: an adult male obtains sexual gratification from repeated exposing his genitals to unsuspecting strangers, usually women and children who are involuntary observers
- He has a strong need to demonstrate masculinity and potency.

3. Fetishism: sexual contact with inanimate articles results in sexual gratification. Most often it is a piece of clothing or footwear.
- Its occurrence is almost exclusive to men who fear rejection by members of the opposite sex.

4. Frotteurism: sexual excitement is achieved by touching and rubbing against a nonconsenting person

5. Sexual Masochism: sexual pleasure occurs while one is experiencing emotional or physical pain.
- The willing recipient of erotic whipping is considered to be masochistic

6. Sexual sadism: sexual gratification is experienced while the person inflicts physical or emotional pain on others.
- Severe forms of this behavior may be present in schizophrenia.

7. Necrophilia: sexual arousal occurs while the person is using corpses to meet sexual needs.

8. Pedophilia: the use of prepubertal children is needed to achieve sexual gratification.
- Pedophilia can be an actual sexual act or a fantasy.

9. Telephone Scatologia: sexual gratification is achieved by telephoning someone and making lewd or obscene remarks.

10. Voyeurism: the achievement of sexual pleasure by looking at unsuspecting persons who are naked, undressing, or engaged in sexual activity.

11. Anilingus – tongue brushing anus

12. Fellatio – inserting penis into the mouth

13. Cunnillingus – tongue brushing vulva

14. Partialism – inserting penis into the other parts of the body.

15. Asphyxiaphilis – suffocating oneself or partner while having sex

+Basic principle for performing a Sexual Assessment

1. be comfortable and at ease with the client

2. Present an open and accepting attitude

3. Be empathetic.

4. Avoid personal values and biases during the interview.

5. Ensure a thorough knowledge base.

6. Establish familiar terminology with the client

7. Encourage the client to verbalize any sexual concerns or emotions.

8. Ask specific, open-ended questions eg. What happened? Tell me about it

9. Support the expression of feelings and validate them

10. Approach emotional or more sensitive questions gradually

+Factors that the subject of sex may not be addressed by nurses:

1. May be embarrassing
2. May not believe that sexuality is part of a presenting problem
3. May feel that discussing sex is an invasion of a client’s privacy
4. May feel inadequately equipped to provide appropriate nursing interventions

Intervention are individualized based on the client’s concern about sexual identity, gender identity, identified sexual disorder, causative factors and clinical symptoms.

Nurses must examine feelings about their own sexuality before they are able to provide care for clients who present with issues related to sexuality.

+NURSING MANAGEMENT

1. Assistance in meeting basic needs
- Adequate rest, exercise and nutrition, and good general physical health promote sexual health. - Clients with health problems or who are elderly are assisted in identifying barriers that interfere with sexual functioning

NCLEX-IELTS

NCLEX a requirement for most BON in the US

A year ago I planned to take the NCLEX and of course everybody knows what it is. The application is very crucial and costly, a single mistake could cost you thousands. so I decided to take my chances from an agency in Manila to process my application since I am working as an instructor, and i cant have the time to process it on line nor thru snail mail so even though it will cost me a little more I did what is convenient for me.

During the NCLEX application process you dont have to do anything just sit back and take time on reviewing which I wasn't able to do. But since I was an instructor I have no time to catch up. Not to mention all those requirements you need to submit during exam periods and the grades that you need to submit before the semester fully closes. well, Going back to the NCLEX Application, you need to submit some of the following requirements but I cannot fully recall haow many times and how can you send this on your own (I have to take the assistance of an Agency in Manila), so i cannot answer you...by the way I submitted my TOR which was authenticated or processed by my school, PRC IDs authenticated in Manila area, PRC certificates or board rating which will be sent thru a courier mail (DHL, Fedex, etc.). and your payments also photo copy of your passport and everything should be valid. they will send you a NBI card or should I say FBI card to be processed thru an NBI personnel. Then everything will be sent to you respective BON.

Well i cant say if you will still take the CGFNS, it depends on your state. well the NCLEX application could take a year but it still depend on the approving state where you apply. By the way there are new ruling posted on your state BON websites which may require you to take the IELTS, Nursing Jurisprudence(i think this is in Texas), and other examinations which may be required before taking the NCLEX, in my case I was required to take the IELTS, so it would be better to check your BON before proceeding to the next step!

Remember that IELTS score should be 7 in speaking and an Over all/ total band score of 6.5.

Recently I decided to take a review for my NCLEX but unexpectedly I wasnt able to apply for my ATT since my NCLEX was not of passing so I decided to take the IELTS and at the same time to process my ATT so that it will not expire last June! - this was a very frustrating time for me! I thought I was ready to take the exam but I am until I pass my IELTS and thank God I did.

I got my ATT and my Schedule of exam on July 20, 2010...9 am!

Hope that i can pass the NCLEX and to all of you too who will be reading this, never stop Praying and for those who took the local Boards this July I congratulate you for facing your fear, the battle is not yet over, the more days pass the more anxiety builds up! So, future colleagues step up and face NCLEX as your next step! As i have said it will eat up your time so apply after passing the locals. Apply for NCLEX, apply for IELTS, apply to a hospital! Then Fly to your destination!

THE CIRCLE OF WILLIS

Why is brain blood supply so important?

The brain represents about 2% of the total body weight in humans, but it receives one fifth of the resting cardiac output and accounts for 15-20% of the body's blood supply. Brain cells die if the supply of blood carrying oxygen is blocked; therefore the brain has the highest priority for the blood. The human body endeavours to supply the brain with an uninterrupted flow of blood.

Blood supply to the brain is carried by two pairs of arteries: the internal carotid arteries and vertebral arteries, which anastomose at the base of the brain to form what is known as the circle of Willis. The right and left vertebral arteries form a single basilar artery at the base of the brain. This then provides the brain blood supply that feeds the circle of Willis.

The carotid arteries and their branches supply the front portion of the brain; and the vertebrobasilar system supplies the rear portion of the brain. These are known as anterior and posterior circulation respectively.

What Relevance is the Brain Blood Supply?

If the supply of blood to the brain is stopped, brain cells die. Ischemic strokes are caused by a blood clot. These can occur with in the brain itself, or in the arteries that make up the brain blood supply. Both situations result in a stroke as the brain is deprived of critical blood.The area of brain blood supply is unique because as a foetus grows, the arteries grow from both theheart and the brain, meeting up in the circle of Willis. Sometimes, the anastomosis (how these arterise join into a network) is not perfect and this can make a person susceptible to problems where plaque can build up at the join. In such cases, the risk of stroke is increased.It's not essential to understand all the parts of the brain blood supply. However, it's worthwhile to understand that this is a complex part of human anatomy.

The Circle of Willis is a vital formation of arteries at the base of thebrain which supplies all thought processes with the necessary fuel.

There is a grouping of arteries near the base of the brain which is called the Arterial Circle of Willis. It is named after a very influential English physician named Thomas Willis, who discovered it and then published his findings in his 1664 work, a seminal peace on the inner workings of the brain entitled, Cerebri anatomi (from the Latin for “Anatomy of the Brain”).

The Circle of Willis

The Circle of Willis or the Circulus Arteriosus is an arterial polygon where the blood carried by the two internal carotid arteries and

the basilar system comes together and then is redistributed by the anterior, middle, and posteriorcerebral arteries. The posterior cerebral artery is connected to the internal carotid artery by the posterior communicating artery.

The anterior communicating artery joins the anterior cerebral arteries of the two hemispheres together. The middle cerebral arteries are connected to the posterior cerebral arteries by the posterior communicating arteries. This anastomosis between arteries is responsible for developing collateral circulation. It provides a safety mechanism. If one of the major vessels becomes occluded within the

Circle or proximal to it, the circle will still provides the brain with continued supply of blood. Thus the circle of Willis is of great use in preventing neurological damage. As long as this circle is successful at maintaining blood pressure at fifty

Percent of normal, no infarction or death of brain tissue will occur in the blocked area and no permanent effects are produced.Smaller arteries arise from the circle of Willis and from the major cerebral arteries. They form four groups which include the anteromedial, the anterolateral, the posteromedial and the posterolateral.

Internal Carotid System

The internal carotid artery divides into two main branches called the middle cerebral artery and the anterior cerebral artery. The middle cerebral artery supplies blood to the frontoparietal somatosensory cortex.

The anterior cerebral artery supplies blood to the frontal lobes and medial aspects of the parietal and occipital lobes. Before this divide, the internal carotid artery gives rise to the anterior communicating artery and the posterior communicating artery.

Vertebral Artery

The two vertebral arteries run along the medulla and fuse at the pontomedullary junction to form the midline basilar artery, also called the vertebro-basilar artery.

Before forming the basilar artery, each vertebral artery gives rise to the posterior spinal artery, the anterior spinal artery, the posterior inferior cerebellar artery (PICA) and branches to the medulla.

Basilar Artery

At the ponto-midbrain junction, the basilar artery divides into the two posterior cerebral arteries. Before this divide, it gives rise to numerous paramedian, short and long circumferential penetrators and two other branches known as the anterior inferior cerebellar artery and the superior cerebellar artery.

Venous Drainage Arteries provide the brain blood supply. The veins take the blood away,after the brain has taken nutrition from it.The prime course of venous drainage of the brain is through cerebral veins that empty into the dural venous sinuses and ultimately into the internal jugular vein.

Cerebral veins are divided into two groups, superficial and deep. The superficial veins usually lie on the surface of the cerebral hemispheres and empty themselves into the superior sagittal sinus. The deep veins drain internal structures and ultimately drain into the straight sinus.

Cerebral veins are thin-walled and valveless. They

are also interconnected by several functional anastomoses both within a group and between the superficial and deep groups. The numerous connections between cerebral veins and dural sinuses and venous systems of the meninges, skull, scalp and nasal sinuses assist the spread of thrombus or infection between these vessels.

The veins of the brain possess no valves, and their walls, owing to the absence of muscular tissue, are extremely thin. They pierce the arachnoid membrane and the inner or meningeal layer of the dura mater, and open into the cranial venous sinuses. They may be divided into two sets, cerebral and cerebellar.

The cerebral veins (vv. cerebri) are divisible into external and internal groups according as they drain the outer surfaces or the inner parts of the hemispheres.

The external veins are the superior, inferior, and middle cerebral.

The Superior Cerebral Veins (vv. cerebri superiores), eight to twelve in number, drain the superior, lateral, and medial surfaces of the hemispheres, and are mainly lodged in the sulci between the gyri, but some run across the gyri. They open into the superior sagittal sinus; the anterior veins runs nearly at right angles to the sinus; the posterior and larger veins are directed obliquely forward and open into the sinus in a direction more or less opposed to the current of the blood contained within it.

The Middle Cerebral Vein (v. cerebri media; superficial Sylvian vein) begins on the lateral surface of the hemisphere, and, running along the lateral cerebral fissure, ends in the cavernous or the sphenoparietal sinus. It is connected (a) with the superior sagittal sinus by the great anastomotic vein of Trolard, which opens into one of the superior cerebral veins; (b) with the transverse sinus by the posterior anastomotic vein of Labbé, which courses over the temporal lobe.

The Inferior Cerebral Veins (vv. cerebri inferiores), of small size, drain the under surfaces of the hemispheres. Those on the orbital surface of the frontal lobe join the superior cerebral veins, and through these open into the superior sagittal sinus; those of the temporal lobe anastomose with the middle cerebral and basal veins, and join the cavernous, sphenoparietal, and superior petrosal sinuses.

The basal vein is formed at the anterior perforated substance by the union of (a) a small anterior cerebral vein which accompanies the anterior cerebral artery, (b) the deep middle cerebral vein (deep Sylvian vein), which receives tributaries from the insula and neighboring gyri, and runs in the lower part of the lateral cerebral fissure, and (c) the inferior striate veins, which leave the corpus striatum through the anterior perforated substance. The basal vein passes backward around the cerebral peduncle, and ends in the internal cerebral vein (vein of Galen); it receives tributaries from the interpeduncular fossa, the inferior horn of the lateral ventricle, the hippocampal gyrus, and the mid-brain.

The Internal Cerebral Veins (vv. cerebri internæ; veins of Galen; deep cerebral veins) drain the deep parts of the hemisphere and are two in number; each is formed near the interventricular foramen by the union of the terminal and choroid veins. They run backward parallel with one another, between the layers of the tela chorioidea of the third ventricle, and beneath the splenium of the corpus callosum, where they unite to form a short trunk, the great cerebral vein; just before their union each receives the corresponding basal vein.

The terminal vein (v. terminalis; vena corporis striati) commences in the groove between the corpus striatum and thalamus, receives numerous veins from both of these parts, and unites behind the crus fornicis with the choroid vein, to form one of the internal cerebral veins. The choroid vein runs along the whole length of the choroid plexus, and receives veins from the hippocampus, the fornix, and the corpus callosum.

The Great Cerebral Vein (v. cerebri magna [Galeni]; great vein of Galen) (Fig. 565), formed by the union of the two internal cerebral veins, is a short median trunk which curves backward and upward around the splenium of the corpus callosum and ends in the anterior extremity of the straight sinus.

The cerebellar veins are placed on the surface of the cerebellum, and are disposed in two sets, superior and inferior. Australia...Another Avenue to Practice Nursing

Mga fellow nurses, gusto kong ishare ang magandang oportunidad na ito!

Filipino nurses are stock-up in the US immigration line waiting for the processing of our priority dates, tama ba?. And it has been a grueling line that does not give assurance when our turn will be. Maraming nagsasabing next month malift na retro, then next year...five years nang ganyan a!!!

So what are we doing about it? Are you going to wait forever? Kaya ako, naghanap ako ng pwede kong mapuntahan...Anyways, not only US need nurses. Another progressive country needing nurses is Australia.
Most of you have heard about the “GAP” program which refers to the IRON program (Initial Registration of Overseas Nurses). It’s a twelve weeks course program required by the Australian Board of Nursing. This course is to be taken in Australia to be registered and qualify in their nursing practice. Para ding pag-aaply na US, kailangan mamuhunan. Marami na kong kakilalangang kumuha ng ganitong course at nandun na sila at nagwowork.

I am very interested with this, and as I search I found a GOOD NEWS. ACFE which is an Accredited Australian Agency that offers IRON program has opened a satellite office here in Manila. The office will process enrollment for the said program. Another GOOD news for those that had been approved by the nursing board to take the IRON course...they offer a 50% scholarship if you plan to take the course this September...wow, that's Php200,000 worth of scholarship, isn't it? Check nyo site nila sa acfe.net.au para mabasa nyo ang sinasabi ko.

To my fellow nurses who had been loosing hope waiting for US opportunities...look around you. There are still so many doors for opportunities. Kailangan lang hanapin natin at simulang pagsikapan.

Rheumatic Heart Disease

Streptococcal (strep) infections are communicable diseases that develop when Streptococcus bacteria normally found on the skin or in the intestines, mouth, nose, reproductive tract, or urinary tract invade other
parts of the body and contaminate blood or tissue.

Rheumatic Fever

—is an inflammatory disease which may develop two to three weeks after a Group A
Streptococcal Infection (such as strep throat or scarlet fever)
Streptococci are a part of normal animal flora!

—It is believed to be caused by antibody cross-reactivity and can involve the heart, joints, skin, and brain

The capsule of Streptococcus pyogenes is chemically similar to that of it's host's
connective tissue, and therefore, is nonantigenic, and it's cytoplasmic membrane has antigens similar to human cardiac, skeletal and smooth muscle.

Epidemiology

—2 – 3 % of people with untreated Group A beta-hemolytic streptococcal infection.
—470,000 new cases and 233,000 deaths each year.
—Mostly in developing countries, among indigenous groups.
—over 15 million people are estimated to have rheumatic heart disease.
—In the US and other developed countries the incidence is low (hygiene and routine antibiotic use)

Predisposing Factors

—AGE – 90% occur between the ages of 5 – 15 y.o.
- Also the AGED, severe cardiac disability and death.
—SOCIOECONOMIC FACTORS – slum, city dweller more than the farmer.
— GENETICS – may appear to develop in household members.

Etiology
—Exact cause remains uncertain!
- bacteria do not grow within the heart and joints.

—2 Theories:

—The body undergoes anallergic response to invading streptococci.
—The host develop an autoimmune response in which streptococcal antibodies attack the host tissue.

—Basis of the theories:

1. RF develops following an URTI by streptococci

2. The devl’p of RF is between 1 - 5weeks, with an average of 18 days. The time our body needs to sensitized an organism and undergo immune response.

3. Since only 2 -3 % devl’p RF after strep. throat, it has been
hypothesized that these people have a greater immunological reaction.

Pathophysiology

Assessment

—Almost always follows a streptococcal infection of the nasopharynx.

Did you experience sore throat lately? How often?

Signs and Symptom

1.Polyarthritis – prominent finding; last hours to days

2.Carditis – common manifestation

3.Relapsing fever – 38o C and episode of normal temp.

4.Subcutaneous nodules – small, painless firm

nodules (knees, knuckles, and elbows); usually in

children; only in first week.

5.Erythema Marginatum – crescent shape lesion with

clear centers “chicken-wire rash”

6.Abdominal pain – may be related to liver

engorgement

7.Sydenham’s chorea – “St. Vitus’ dance”; late stages,

usually in girls; Involuntary grimacing and jerky,

purposeless movements.

8.Malaise, weakness, weight loss, and anorexia – As a

result of fever, pain, and the general debilitation

Diagnostic Measures

—There is no single diagnostic feature identifies rheumatic fever.

JONES CRITERIA

- gauges the probability of the presence of rheumatic fever in an individual.

Premise:

1. Diagnosis requires two major manifestation

2. One major manifestation and two minor manifestation

Previous rheumatic fever or inactive heart disease

Medical Intervention

—Control and alleviate infecting streptococci if still present.
—Protect the heart against the damaging effects of carditis.
—Relieve joint pain, fever, and other symptoms.

—Typical intervention:

1. Chemotherapy with penicillin, salicylates, and steriods
2. Bed rest
3. Proper diet

Pharmacologic Intervention

—PENICILLIN – for 10 days ff. the onset of rheumatic fever.
—ERYTHROMYCIN – if allergic to penicillin
—Prophylactic doses of same med is given to prevent further attacks
—Monthly injections of Longacting Penicillin must be given for a period of 5 years in patients having one attack of Rheumatic fever
—SALICYLATES – to control fever and to relieve joint pain.
—Aspirin – give with food to reduce gastric irritation
—STERIODS – relieve inflammatory symptoms; prevent further scarring of tissue and may prevent development of sequelae such as Mitral stenosis
—CARDIAC GLYCOSIDES (‘digitalis’) and DIURETICS

Nursing Dx and Interaction

—Alteration in Comfort
—Activity Intolerance – Bed rest (reduces strain on the heart and reduces metabolic needs)

* Temp is normal without salicylates

* Resting pulse (adults) <100>

Prognosis and Complication

—With antibiotic therapy, the prognosis is generally good. (only 1 – 2 % die from initial attack; acute myocarditis)
—Laboratory and clinical signs subsides within one to two months following therapy.
—Some develop residual heart damage:

MITRAL REGURGITATION and/or STENOSIS AORTIC REGURGITATION
